Agassiz Village Summer Camp Program Forms


This is the list of various forms and documents needed to register your child for our summer camp program. Each form and document has a short description indicating what it is and why/when you need to submit it.

Camper Registration Form – use this form to register for any Agassiz Village summer camp programs. You can download a hard copy of this form by clicking on the name of it or clicking on the following link to complete and submit it online: http://icebrrg.com/Public/ViewForm.aspx?formID=63733.

Free/Reduced Meal (FRM) Form – Agassiz Village participates in the State of Maine Summer Food Service Program which helps us to cover some of our summer food costs and, hence, keep our tuition rates low. We are asking all parents registering their children for our summer camp to complete this form so we can comply with requirements of this program. Please download this form, print, complete, sign and send it back to us to go along with your deposit and camper registration form.

Financial Aid Form – This form is for families who would like to apply for Agassiz Village Financial Aid. Our campership fund aims to provide assistance based on need to families registering their children for one of our programs. Please download, print, and sign this form and send it back to us along with your camper registration form. Please make sure the information on the form is correct. We may also be requiring additional proof of income if you will be applying for full campership. We will notify you separately if that will be the case.

Health History Form – After your camper registration is accepted, please complete this form and submit it to us no later than two weeks prior to the camp start date. Please also be sure to have it reviewed by your doctor/physician. Please note that if Health History and/or Physical Exam forms are missing on camp registration day, your child will not be able to attend the camp session.

Physical Exam Form – Please have this form completed and signed by your doctor/physician and sent to us at the address provided on the form. Please note that if Health History and/or Physical Exam forms are missing on camp registration day, your child will not be able to attend the camp session.

Parent Information Handbook – This handbook will provide you with important information and details on your child’s stay at Agassiz Village. It will provide you with contact information while camp is in session, what-to-bring and what-not-to-bring lists, general policies and procedures, and a lot of other pertinent information. Please read through it carefully and let us know if you have any further questions. The link to this document will be posted in the beginning of February.
